Detailed records provided guidance to ensure people received safe, person-centred care. Risks to people had been assessed and care plans were in place to help mitigate risks as far as possible. Staff worked with all agencies involved in people’s care for the best outcomes and smooth transitions when people moved services or attended appointments. They monitored people’s health to support healthy living. Information was accessible to involve people in decision making about their lives Staff ensured people understood their care and support to enable them to give informed consent. People were treated with kindness and respect. Staff protected their privacy and dignity. People were protected and kept safe. Processes were in place to assess risks associated with people’s care and their home environment. There were enough staff with the necessary skills, qualifications and experience. Rosters were well-managed, so people received consistent care from staff they knew. Staff managed medicines well and involved people in planning any changes. Effective systems were in place to monitor the quality and safety of the service. The service had a good learning culture. The registered manager investigated incidents thoroughly and people could raise concerns. The registered manager was visible, knowledgeable and supportive, helping staff develop in their roles. Staff and leaders demonstrated a listening culture and were open to learning and improvements.”
