Risk assessments and care plans were not always accurate or kept up to date to reflect people’s changing needs.”

“The service was not always effective in supporting people to maintain or improve their independence. There was limited evidence of structured rehabilitation planning or review of outcomes.”

“People were treated with kindness, and staff were observed responding to people’s immediate needs. However, personalised care planning was inconsistent, and people and relatives were not always meaningfully involved in decisions about care, goals or restrictions.”

“Leadership and governance were not effective. The service had experienced significant management change and quality assurance systems were not effective in identifying or addressing known risks. Several issues identified at this assessment had been recognised previously but had not been resolved, increasing risks to people.”
