“Date of Assessment: 31 March to 22 April 2026. Fulford Care and Nursing Home provide accommodation with nursing and personal care for up to 74 people. At inspection 58 people were receiving a service. People’s needs included clinical nursing care, end of life support, and people living with dementia. Two people with healthcare needs had a learning disability.”

“This was a risk-based assessment prompted in part by concerns raised to us about people’s safety and how the service was managed.”

“The service has been rated good. This inspection found one breach of legal regulations in relation to good governance.”

“Quality monitoring processes were inconsistent and failed to identify concerns found at inspection. The service had not ensured the effective application of the Mental Capacity Act (2005) and the Oliver McGowan Code of Practice. Governance and oversight systems were not effective in identifying and mitigating risks to people’s care. Risks were not always identified or managed safely, and feedback indicated some inconsistency in staff practice. Processes did not always support a person-centred approach to end-of-life care planning.”

“People told us they felt safe and were protected from abuse. Staff understood safeguarding procedures, and the provider demonstrated a learning culture by using incidents and accidents to improve practice. Medicines were managed safely, and there were enough staff to meet people’s needs.”

“People’s needs were assessed and used to inform care planning, with care plans reflecting their health, communication needs, and personal preferences. Staff worked with health and social care professionals to support people’s wellbeing and respond to changes in their health. We observed positive interactions between people and staff, and people were treated with dignity and respect. Staff supported people to maintain their independence and make choices about their care.”

“The service was responsive to people’s needs, and people and those important to them participated in decisions about their care. Information was provided in accessible ways, people knew how to raise concerns, and there were opportunities for social engagement and activities.”

“Leaders were visible within the service, and staff felt able to speak up. Leaders responded positively to feedback and took prompt action to strengthen quality monitoring systems and improve compliance.”

“We have asked the provider to submit an action plan in response to the concerns identified during this assessment.”
