“People were not always protected from the risk of harm. Information about people’s care plans and risk assessments were not always up to date and accurate. There were shortfalls in care planning and risk assessment processes. We also identified concerns relating to medicines management. This meant people were not being adequately protected from the risk of receiving unsafe and inappropriate care.”
