“The provider now had effective systems to assess how the service was operating and plan improvements where needed. There was a range of monitoring and auditing systems used by the provider. This information was used to create a central action plan, which had named staff responsible and a timescale for completion. The plan was regularly reviewed by the registered manager and regional director to ensure actions had been completed.”

“People were supported to manage identified risks to them, including ensuring people were regularly re-positioned to prevent pressure injury, supported to manage the risk of falls and regular well-being checks. Staff worked with all agencies involved in people’s care for the best outcomes and smooth transitions when moving between services. The service was clean and well maintained throughout. The provider had systems in place to share learning from incidents in other services.”
