“Care was planned and delivered using up to date assessments and evidence-based guidance. Staff worked well with other professionals to keep people well and respond quickly when health needs changed. People had enough to eat and drink and were supported to make healthier choices. People were treated with dignity and kindness and staff respected their preferences, beliefs and backgrounds. People were involved in decisions and felt listened to. Staff recognised individual communication needs and provided information in accessible ways.”

“The service worked flexibly to meet people’s changing needs and ensured continuity of care through strong partnership working. People and relatives said concerns were addressed quickly and feedback was used to improve the service. End of life care was compassionate and reflected people’s wishes. Leadership was visible and inclusive. Staff felt well supported and part of an open and positive culture. Governance systems were effective and helped the registered manager identify and act on improvements. The service continued to learn and develop, involving staff, people and external partners to improve outcomes.”
