People experienced good continuity of care from a consistent staff team, helping to build trusting relationships. Risks were assessed and managed in partnership with people and their families, and care plans were detailed, person-centred and regularly reviewed, particularly following changes in need or hospital admission. Medicines were managed safely where support was required, and infection prevention and control practices were followed. There was a strong learning culture, with incidents, accidents and safeguarding concerns reported appropriately and used to drive improvement.”
