The provider involved people in planning and reviewing their care and treatment, ensuring support was tailored to what mattered most to them and reflected their desired outcomes. While no one was receiving end-of-life care at the time of the assessment, the provider had begun to strengthen its approach to advance care planning by exploring people’s wishes and preferences for their future end-of-life care.”
