“At our last visit to this service on 9 January 2023, we found people were at risk of avoidable harm because risk management was not effective. The provider had failed to have systems and processes in place to manage risks. This was a breach of regulation 12 (Safe care and treatment). We also found at that inspection that safe recruitment practices were not in place which was a breach of regulation 19 (fit and proper persons employed). The third breach at the last inspection was a breach of regulation 17 (Good governance) because the provider’s quality assurance systems and processes were not effective and had not enabled them to assess, monitor and improve the quality and safety of the service.”
“At this assessment we found some improvements had been made, however, there was further work to do, and the provider remained in breach of regulation 17 (good governance).”
