“Date of assessment 16 February and 26 February.”

“Pear Tree Residential care home is registered to provide Accommodation for persons who require nursing or personal care for up to 20 people. This includes older people and people living with Dementia. At the time of our assessment 17 people were using the service. The assessment included 2 on site visits, with further information and evidence reviewed remotely. The assessment was completed to follow up on action we told the provider to take at the last assessment.”

“The last assessment for this service was completed in August 2022 and there were breaches of regulation identified around safe care and treatment, person centred care, staffing and governance of the service. During this assessment we found breaches of the legal regulations in relation to safe care and treatment, fit and proper persons, staffing, mental capacity and governance of the service.”

“ ”

“The service has been rated requires improvement in the well-led domain at the last 5 inspections dating back to 2016 and rated inadequate in the well-led at the last inspection in 2022. The service has been rated requires improvement in the well-led domain at the last 5 inspections dating back to 2016 and was rated inadequate in this domain at the most recent inspection in 2022.”

“ ”

“The provider had failed to make improvements around their governance processes and there was a lack of effective systems and processes to monitor the quality and safety of the service. This included a lack of safety checks around fire, equipment and the environment. Where external contractors had identified issues, the provider had failed to take timely action to rectify these.”

“Safe recruitment checks were not always completed for staff to ensure they were suitable to work with vulnerable adults. Staff had not always received the required training and updates to ensure they were skilled and competent in their roles.”

“Risk assessments and care plans were not always in place or reviewed to provide appropriate guidance to staff on how best to support people and reduce potential risks. Accidents and incidents, including falls were not appropriately documented, reviewed or managed to mitigate future risk.”

“Where people did not have capacity to make decisions for themselves, relevant mental capacity assessment and best interest decisions had not taken place to ensure decisions made were in their best interest and the least restrictive option.”

“Improvements had been made around the management and storage of medicines, and people were receiving their medicines safely. We found the provider was seeking feedback from people around the quality of food provided and family and visitors were able to provide written feedback on questionnaires. However, no other feedback was sought from people, around their care, support or environment.”

“Improvements had been made around the storage and staff’s use of Personal Protective Equipment (PPE). However, further maintenance in one of the bathrooms was needed to ensure surfaces could be effectively cleaned.”

“In instances where CQC has begun a process of regulatory action, we may publish this information on our website after any representations and/or appeals have been concluded, if the action has been taken forward.”
