“People’s care plans did not always have clear, consistent and up to date information to enable staff to support people appropriately. Risks to people’s safety were not always managed effectively, and documentation including assessments were not always reflective of peoples risks and support needs. Safe recruitment processes were in place, however, the service had struggled with recruitment of staff, as a result staffing was supplemented by a large number of agency staff.”

“People received their medicines on time and in a safe way. For people who were prescribed PRN (as and when required) medicines, protocols were in place to ensure people received these appropriately. Where people missed medicines, documentation did not always evidence what action was taken. Although newly implemented governance systems were in place, these had not yet been embedded into practice and had not identified some of the shortfalls found during the assessment.”
