Complex home care services

Voyage Specialist Healthcare was founded in 2005 and provides complex continuing healthcare services for adults and children who choose to live in their own homes. Our healthcare professionals provide highly specialised care for clients with complex physical healthcare needs who may also require support with learning disabilities.

Our complex continuing healthcare services are most suited to individuals who:

-Have physical or learning disabilities and require long-term support in the home.

-Have been discharged from hospital and have significant nursing and clinical care needs that need to be continually monitored. 

-Are in hospital wards such as intensive care or high dependency units and are due to return home, faced with a long-term recovery process. 

-Suffer from degenerative conditions and have on-going care needs.  

We develop the optimal balance between the non-clinical and clinical needsof our clients and ensure they are supported in defining and achieving outcomes that truly matter. Clients play a pivotal role in the delivery of their care and we encourage their involvement in a range of activities including:

-Defining the person centred care package, service rotas and the profile of staff to be involved 

-Interviewing candidates to ensure a suitable match with their requirements

-Input into the on-going assessment and review of staff

Every client is provided with a dedicated experienced and registered nurse and/or behavioural therapist who ensures that our healthcare assistants are properly trained and that our clients ongoing complex home care needs are being met.
